Lifetime Membership
Our Lifetime Members are the heartbeat of the 11-99 Foundation and are sincerely and strongly committed to helping us reach out and assist the men and women of the California Highway Patrol. Thank you to all our donors who joined or upgraded in 2019!
Become a member by Applying Online and paying the one-time fee, or if paying by check, please download and complete the Membership Application Form. You must be at least 25 years old to apply. Read the Membership Terms & Conditions for more info. NOTE: Background checks may take up to 10 business days, and your membership items should be received within 8 weeks of submitting your application.
COVID-19 Delays of Membership Items & Orders: Due to factory closures as a result of COVID-19, some membership items have become temporarily unavailable, causing delays in fulfillment of membership packages and/or store orders. We appreciate you bearing with us if it has taken longer than expected to receive your items, and will provide updates and fulfill orders as soon as we possibly can. Thank you for your patience and understanding.
If you are an existing member, and are interested adding your spouse/partner to your membership:
CLASSIC & BRONZE MEMBERS: Please fill out and sign this form and submit with payment (mail in a check along with the form, or call the Fullerton office to pay by credit card and email the form to applications@chp11-99.org. (Reminder: $1,000 to add spouse to Classic membership; $1,500 to add spouse to Bronze membership)
SILVER, GOLD, & PLATINUM MEMBERS: Please fill out and sign this form and email back to applications@chp11-99.org for processing, or mail the form to our Fullerton office. No additional payment is required as spouse/partners are included in these membership levels, however background check approval must still take place before spouse/partner’s membership items are ordered.

You would owe the difference between your current membership level and desired membership level. To upgrade, please download and complete the Membership Upgrade Form and email to the appropriate staff listed below, or mail in to our Fullerton office.
